Study Finds Delayed Autism Diagnosis in Paraguay Linked to Late Concerns and Strong Verbal Skills

A recent study has shed light on the demographic and clinical factors that contribute to the age of autism diagnosis in Paraguay, marking a significant step in understanding the challenges of early autism identification in the country. The research, published in the journal Autism, explored the experiences of carers of autistic children and aimed to discover why some children receive their diagnosis later than others.

Differences in social communication and repetitive behaviors are hallmarks of autism, a lifelong condition. Early diagnosis is crucial for improving outcomes through timely intervention. But the age at which children receive an autism diagnosis varies widely, particularly in low- to middle-income countries like Paraguay, where healthcare resources can be scarce.

The study involved 176 carers of autistic children under the age of 18 living in Paraguay. Researchers used a detailed analysis to identify several factors that impact the age of diagnosis. Two main findings emerged as significant: the child’s age at the time of carers’ first concerns and the child’s verbal skills at the time of diagnosis.

The older the child was when carers first expressed concern about their development, the later the autism diagnosis occurred. This suggests that delays in recognising early signs of autism could push back the diagnostic process. Additionally, children with more advanced verbal skills were often diagnosed at a later age, possibly because autism in children with better language development may be harder to identify, leading to delayed intervention.

The study highlighted a gap between when carers first noticed developmental concerns and when a formal diagnosis was made. On average, carers raised concerns when their child was around 26 months old, yet the mean age of diagnosis was 39 months. This 13-month gap, although smaller than in some other Latin American countries, indicates that barriers to early diagnosis remain, such as access to specialised services and long waiting times for assessments.

In other countries like Venezuela, the gap between concern and diagnosis can extend to over 30 months, underscoring the disparities in healthcare systems across the region. Paraguay’s relatively shorter gap, however, still points to the need for improvements in diagnostic pathways.

Interestingly, the child’s verbal ability emerged as a strong predictor of diagnostic timing. Children who displayed more advanced language skills, such as using complex sentences, were often diagnosed later. This may be due to the fact that children who speak well can mask other signs of autism, leading to delays in identification. However, verbal ability alone does not negate the presence of autism, as many children with autism still display atypical social communication behaviours.

The study’s findings emphasise the need for greater awareness among carers and professionals about the social communication differences in autism that extend beyond verbal ability. Educating the public about these nuances could lead to earlier identification and intervention.

This study is the first of its kind in Paraguay, focusing on the predictors of autism diagnosis in a country where autism prevalence remains unclear. It provides a foundation for future research and highlights the urgent need for reforms in healthcare policies to improve access to diagnostic services.

The findings suggest that public health initiatives in Paraguay should focus on educating carers and healthcare professionals about the early signs of autism. Raising awareness and increasing the availability of diagnostic services outside the capital could lead to earlier identification and better outcomes for autistic children.
